Yes that is how it should go. I don't think other than maybe opening day a manager would go with most popular, and if I was a manager I would go with the best pitcher. Then you adjust the rotation around that. Definitely have seen it play out where some one gets puched back, for example, because they want a lefty in between 2 righties.

I wouldn't have said inept but more of babying the staff. 13, even at times they had 14 pitchers on the staff and still teams used position players. I do not dislike that teams would on occasion use one to pitch in a blowout. But when guys like Chris Gimenez and La Tortuga get 7-8 appearances in one season that's ridiculous. RP need to be able to pitch more than one inning in a game. They also need to be able to go back to back days if it's only one inning. Maybe not related but I miss the days of the LOOGY. They could go 3, 4, 5 days in a row. Not innings eaters, but the three batter rule is also dumb Manfraud ball.
